Name of the allegedly infringed copyrighted work or, if the notification covers more than one, a list of all works on that site covered by that notification. The best way to help us find the items quickly is to include YouTube URLs in the body of an email.

That the service provider can most likely use to contact the complainant, such as an address, phone number, and, if possible, an email address.

There must be proof that the person complaining truly believes that the material is not being used in the way that they claim it is or that the law prohibits it.

A sworn declaration that the information in the notification is correct and that the person filing the complaint is authorized to speak on behalf of the owner of an exclusive right that is allegedly being violated. (It is worth noting that Section 512(f) states that someone can be sued for damages if they knowingly and materially lie about something being infringing. That is to say, DON’T LYE!
